These are the most frequently required forms.
Please download the appropriate form(s) and complete them prior to your child's riding lesson.
If your child will be participating in Therapetic or English riding lessons on a regualar basis, these are the form(s) you need to download.
- Rider Liability Release Form must be signed by a parent or guardian and brought to the first riding lesson.
- Rider Medical History must completed for riders who have special needs and signed by a physician, pt, or ot and brought to the first riding lesson.
If your child will be participating in an event with Scouts or another youth group, this form must be signed by a parent or guardian and brought to the event.
Riders with special needs must also bring the Rider Medical History Form (above) signed by a physician, pt, or ot.
Volunteers need to complete and bring the Volunteer Liability Release Form at or before their first session.
